Card Alert/Fraud Report Cancellation: If you’ve recently received a card transaction alert notice and had verified the transaction(s) in question are fraud, but then confirmed the reported transactions are actually valid, please complete the following form in order for the fraud activity status on the alert to be cancelled. Once this form has been submitted, your card will then be reactivated.
